• Print and complete donation form below.

Teutopolis Education Foundation Memorial Donation Form

• Deposit this at the memorial service or send to the Teutopolis Educational Foundation.

• The Teutopolis Educational Foundation will send a notification of receipt of donation to the loved one’s family.

• The Teutopolis Educational Foundation will send a thank you & IRS acknowledgement to the donor.

• The person for whom the donation was made will be added to the list of names memorialized on the district website.

• The name will be added to the list no matter the size of the donation. All assistance is truly appreciated.
